On examination there is granulation tissue at the bony-cartilaginous junction of the floor of the external auditory canal, the tympanic membrane is intact, and today he has a new House-Brackmann grade IV left facial weakness. Inflammatory markers are raised and he is afebrile. How would you approach this patient?
