A 46-year-old Bangladeshi labourer is brought to the emergency department with 14 hours of severe generalised abdominal pain that began suddenly in the epigastrium. He is drowsy, blood pressure 82/50, pulse 128, respiratory rate 30, temperature 38.4, and the abdomen is board-rigid. An erect chest radiograph shows free gas under both hemidiaphragms and the lactate is 5.6 mmol/L. He takes diclofenac daily from a pharmacy without prescription and speaks only Bengali. How would you manage this patient?
What the panel is assessing
The panel is testing whether you can run a septic shock resuscitation and a definitive operative plan in parallel rather than sequentially, and whether you keep control of timing, source control and consent in a patient with a language barrier.
