You are the on-call consultant gastroenterologist in a tertiary hospital at 02:00. A 55-year-old man with known hepatitis C cirrhosis, Child-Pugh C (score 10), is brought to the emergency department having vomited approximately one litre of fresh blood at home. His blood pressure is 82/48 mmHg, heart rate 122, he is drowsy but rousable, haemoglobin 71 g/L, platelets 62, INR 2.1, bilirubin 78 micromol/L and creatinine 148 micromol/L. The emergency physician has started crystalloid and is asking whether to transfuse four units of packed cells and correct the INR with fresh frozen plasma before calling you to scope. How would you manage this patient?
What the panel is assessing
The panel is testing whether the candidate can lead a time-critical resuscitation in a decompensated cirrhotic, apply a restrictive transfusion strategy and modern coagulation understanding rather than reflex product transfusion, and sequence airway, drugs, antibiotics, endoscopy and pre-emptive TIPS correctly. It also probes whether the candidate can direct a multidisciplinary team under pressure rather than simply describe endoscopic technique.
