You are the on-call consultant urologist at 02:00 in a tertiary hospital. A 38-year-old expatriate labourer with a solitary right kidney (left nephrectomy for trauma eight years ago) is brought to the emergency department with two days of right loin pain, rigors and reduced urine output. He is confused, temperature 39.4 C, blood pressure 82/48, pulse 128, lactate 4.6 mmol/L, white cell count 21, creatinine 348 micromol/L and potassium 5.9 mmol/L. A non-contrast CT KUB shows a 9 mm mid-ureteric stone of 1100 Hounsfield units with moderate hydronephrosis and perinephric stranding. How would you manage this patient?
What the panel is assessing
The panel is testing whether you recognise obstructed infected upper tract as a time-critical urological emergency and can prioritise resuscitation and drainage over stone treatment. They also want to see safe consultant-level decision-making about the route of decompression in a septic, coagulopathic patient with a single kidney.
