You are the on-call consultant in a private hospital in Dubai at 21:00. A 54-year-old man with a BMI of 38 and a short thick neck is being induced for an emergency laparotomy for a perforated viscus by a specialist anaesthetist who calls you in. After a modified rapid sequence induction with propofol and rocuronium, direct laryngoscopy shows a Cormack-Lehane grade 3b view, two intubation attempts have failed, a second-generation supraglottic airway has been inserted twice without an adequate seal, and face-mask ventilation is now impossible. Saturation is 78 per cent and falling, and the heart rate is 48. How would you manage this patient?
What the panel is assessing
Whether the candidate can run a time-critical, protocolised airway crisis under pressure rather than continuing repeated intubation attempts, and whether they will actually commit to a surgical airway with correct technique and language. The panel is also watching for team leadership and declaration of the emergency.
